Abstract

Malignant paratesticular tumors are rare, with sarcomas representing the most common subtype and typically spreading via local progression rather than distant metastasis. We describe a case of paratesticular high-grade dedifferentiated liposarcoma with metastasis to the right corpus cavernosum in a patient with concurrent chronic lymphocytic leukemia. This case is notable for its aggressive histopathology and atypical pattern of spread. Management consisted of radical orchiectomy with subsequent resection of the crural lesion followed by adjuvant radiation, though optimal treatment strategies remain unclear. Recognizing atypical patterns of spread is critical to guiding appropriate surgical planning and postoperative management in complex cases.
